What is Vaginoplasty?

Vaginoplasty for trans women, is a procedure aimed at creating a vaginal cavity in the perineum (between the scrotum and annum), maintaining tactile and erogenous sensitivity to give a very similar result to a vagina in a biological female.

It enables sexual intercourse; and creates the clitoris, labia majora and minora. There are different techniques used. The technique employed depends mainly on the size of your penis. The two that we apply the most are penile skin inversion and penile skin grafting. In secondary cases, or with a very small penis, we apply a colon vaginoplasty procedure.

A 10 to 16 cm vagina is attained and in 90 per cent of the cases with orgasm and normal-like appearance. It´s an absolute change.

The goal is to create a vaginal cavity that is indistinguishable from that of a biological woman, allowing for tactile and erogenous sensitivity, minimal scarring and full functionality.

Preparing for Surgery

In the pre-surgical assessment appointment, we will analyze jointly the most suitable option for you, as well as your expected outcomes. Permanent hair removal may be required in penile and testicular areas prior to surgery.

Female hormone therapy supervised by an endocrinologist for at least the last 12 months.  Suspend hormone therapy 4 weeks before surgery (estrogens increase the risk of deep vein thrombosis). You will notice physical and emotional changes during this time. Hormone therapy is resumed 2 weeks after surgery.

  • Do not take aspirin products one week before surgical procedure.
  • Discontinue the use of hormone contraceptives two weeks in advance.
  • Do not smoke for two weeks before surgery and during the 2 weeks after.
  • Do not drink alcoholic beverages within 72 hours before surgery.
  • Do not engage in any travel 5 days before and 7 days after your surgery.
  • You must be admitted to hospital one day before surgery for an intestinal cleanse with laxatives and antibiotics, for greater safety during the conduction of the surgical procedure.
  • Permanent hair removal is required all over penile and testicular areas.
  • Orchiectomy is required, in case you have not had it, this procedure will be performed during surgery.

Aftercare

At discharge, you will be provided with a detailed list of the required specific care measures you must follow. Generally speaking:

  • You should use a vaginal dilator twice a day for at least 15 minutes (we have dilators available; they are sold separately)
  • You must use vaginal dilators during sleep for up to 2 years after surgery.
  • Resume hormone therapy 2 weeks after surgery.
  • Maintain a healthy diet.
  • Take your medication as indicated.
  • Observe strict hygiene of surgical area.
  • Do not exercise until indicated by your doctor.
  • Avoid vaginal intercourse for 2 months after surgery.

Follow up

We will conduct an evaluation session one week after surgery, one month after; and within six months to a year to monitor permanence of results. If you’re coming from a different town, you must stay in Mazatlán for approximately 2 weeks after surgery.  The longer you stay the better your control you will have.  You must maintain constant communication with us afterwards, whether by phone or by teleconference.  Ideally, you should attend a consultation with us in approximately one year after surgery.

If necessary, you could visit us at any time during your recovery.

Our staff will maintain constant communication to inquire how you have been evolving and to detect any potential incident in a promptly manner; in addition to answering your questions and providing you with follow-up care during your recovery process.

What to Expect

The exceptional results attained with this procedure can be observed immediately after surgery, and a definitive outcome can be seen 6 to 8 weeks after, since during this period the areas involved could experience swelling; which is expected to occur and completely normal.

Use of dilators is generally for life in the case of penile inversion and graft procedures. Although their use goes down to one time a day over time. With colon vaginoplasty, sometimes abundant foul-smelling stools are expected to occur, which decrease until 6 months after surgery.

Attained vaginal depth runs from 8 to 15 cm with skin inversion techniques, and up to 18 cm with colon vaginoplasty. If you do not perform vaginal dilations you run the risk of losing depth.
